Staff who drive a vehicle transporting children shall disclose any traffic violation that occurred five years prior to or during employment or assignment as a driver. Staff 5, who transported children on 12/12/2025, did not have a driver statement until 12/15/2025.
Plan of Correction: The driver record was obtained 12/15/2025.

Staff shall wash hands with soap and running water after a diaper change. Staff did not wash hands after changing a child?s diaper.
Children's hands shall be washed with soap and running water after toileting. Staff did not wash an infant's hands after a diaper change.
