Inspector Notes

An unannounced renewal inspection was conducted May 31 2022 between 10:00 and 11:00am. There were 2 children and one provider child in the direct care of the provider. 2 child records, 2 household records, and the physical plant of the home was inspected and reviewed. Based on household record review,the provider did not ensure that each household member had a Current Report of Tuberculosis Screening every two years from the date of the first screening.

Household member #1 and #2 Tuberculosis Screening records are dated October 28, 2019.
Plan of Correction: Provider submitted Report of Screening for both household members dated 6/3/2022

Based on record review, 1 of 2 children's records reviewed did not contain written agreements signed by the parents by the end of the first day of care.

Evidence: Child #1 Record did not contained the signed written agreements.
Plan of Correction: Provider stated that she will get the forms from the parents.
