During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted 11/25/2024 from 11:10am- 1:00pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; totaling 10 points. The inspector reviewed 3 children's records, 1 caregiver/provider record, and 1household members record onsite. This inspection included document review, interviews, observations, and a tour of the home.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-60-B
A child's record shall contain the following information: Name, office address and telephone number of the child's physician; name, address, and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; names of persons other than the custodial parents who are authorized to pick up the child.

The record for Child #1, enrolled for one week, was missing the required physician information, two emergency contacts, and the authorized pick up. The record for Child #2, enrolled for one week, was missing the physicians name, an address for emergency contact #1, and a second emergency contact. The record for Child #3, enrolled for more than one month, was missing the phone number for emergency contact #1 and the authorized pick up.

Before the child's first day of attendance, parents shall be provided in writing the following information: Policies for the administration of medication.
