Inspector Notes

An unannounced monitoring inspection was conducted on February 4, 2026 in response to a self-report received by the licensing office on January 12, 2026 relating to infant care. Violations have been cited related to noncompliance resulting in an injury to a child while under the supervision of the facility that required outside medical treatment. Please see the violation notice for more details. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to injury reporting and file retention and are documented on the violation notice issued to the program.

Records and reports on children shall be maintained and made accessible for two years after termination of services unless specified otherwise. Child 1's file was returned to the parent at the parent's request. The child's file could not be reviewed as part of the investigation.

Standard 8VAC20-780-570-D
Infants shall be fed on demand or in accordance with parental instructions. The parents of Child 1 (age 6 months) provided instructions that the infant should be fed every two hours. Reportedly, Child 1 had a bottle at 12:23 p.m. on January 8, 2026. Child 1 took a nap from approximately 3:22 p.m. to 4:08 p.m. A bottle was not provided at approximately 2:23 p.m. when due.

Standard 8VAC20-780-80-B-2
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. On January 8, 2026, a child was taken to the emergency room after sustaining an injury at the center. Center staff did not notify the Department as required.
