Plan of Correction: Daycare will immediately obtain the proper signature for Child #1's immunization record from the child's parent and healthcare provided. Additionally, going forward, administrative staff will review all student files to ensure that all immunization records are complete.

Plan of Correction: Daycare will immediately reinforce our menu posting procedure. Staff responsible for morning snacks will be trained to update the menu board daily and document any substitutions promptly and clearly. The director or designee will verify menu accuracy each morning to ensure ongoing compliance.
