During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 1/8/26, as a part of probation. The Department issued a Special Order on 11/10/25 to place the licensee on probation for a period of three months and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The inspector reviewed compliance in the areas listed above. There were eight children present and three staff. The inspector reviewed zero children?s records and four staff records on-site. This inspection included document review, a tour of the facility, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
