Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on 1/15/2025 and completed on 1/15/2025. The inspection started at 11:30 am and concluded at 12:15 pm. Home-based: There were 3 children present and 1 vendor; the point total was 6. The inspector reviewed 3 children?s records, 1 vendor record, and 3 household member(s) records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/22/2025.

In areas used by children of preschool age or younger, the following shall apply: Electrical outlets shall have protective covers.

Three outlets on a surge protector near the children's table were missing protective covers.
Plan of Correction: I move the electricy outlets out of the children's area.
