During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/8/2025. The on-site inspection began at 9:25am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 58 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 did not have documentation of their 18 month immunizations.
Plan of Correction: The center will obtain updated immunization records.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #3 was due for an updated screening four months ago and Staff #4 was due for an updated screening six months ago. The updated screenings have not yet been completed.
Plan of Correction: The center will ensure the staff submit updated TB screening documentation.

Repeat Violation

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The first step on the outside playground unit had a rubber coating that was peeling off causing increased risk of a child tripping.
Plan of Correction: The center will have the stair coating repaired.
Standard 8VAC20-780-280-B
Hazardous substances must be in a locked place that prevents access by children. In the infant classroom, diaper cream (labeled, keep out of reach of children) was found near the changing table, out of reach of the children.
Plan of Correction: The diaper cream was moved to a locked area during the inspection.
