22.1 EARLY CHILDHOOD CARE AND EDUCATION

Inspector Notes

A monitoring inspection was conducted on April 27, 2023. The inspector was on site from approximately 2:00 pm until 3:30 pm. The director was present and assisted with the inspection. There were 53 children present, ranging in ages from seven years to twelve years, with six staff supervising. Children were observed reading and working on reading strategies, playing a math game, interactive tablet activities, and playing in the gym with hula hoops, scooters and balls. Later children ate a meal in the cafeteria. Handwashing was conducted and tables were cleaned. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. Six staff records were review virtually on May 1, 2023.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
Based on review of five children?s records, the center did not obtain documentation of immunizations before the child?s attendance.

Evidence:

The record of child #4, enrolled 09/26/2022, contained an immunization record dated 01/04/2023.
Plan of Correction: Per the director: We will check at enrollment to make sure we have all required documentation.
Standard 8VAC20-780-140-A
Based on review of five children?s records, the center did not obtain documentation of a physical before the child?s attendance or within 30 days after the first day of attendance.

Evidence:

The record of child #4, enrolled 09/26/2022, contained documentation of a physical examination dated 01/04/2023.
Plan of Correction: Per the director: We will check to ensure all paperwork is obtained in the required timeframe.

Based on review of documentation and interview, the center did not obtain an annual fire inspection report.

Evidence:

The most recent fire inspection report was dated 10/11/2021, exceeding the annual requirement. The director stated that she did not have a copy of a current fire inspection.
Plan of Correction: Per the director: I will ask the school to provide a copy of the current fire inspection.

Based on observation, the center did not ensure hazardous substances were keep in a locked place using a safe locking method that prevents access by children.

Evidence:

In the art room where children were present, there were several cans of paint with a warning label that stated to keep out of reach of children and hazardous to one?s health.
Plan of Correction: Per the director: I will make sure school supplies that need to be locked are removed from areas we use.

Based on review of the emergency drill log and interview, the center did not practice lockdown procedures at least annually.

Evidence:

The drill log did not have a lockdown practice drill documented for the 2021-2022 school year. The director stated a lockdown drill had not been conducted.
Plan of Correction: Per the director: I will have a practice lockdown drill before the end of the school year.
