Inspector Notes
An annual subsidy inspection was initiated on 11/2/2023 and concluded on 11/2/2023. There was one child present with one vendor supervising totaling one point. The inspector reviewed compliance in the areas of General Qualifications,Building or Home Maintenance,Supervisionand Ratio Requirements, Programs, Special Care Provisions & Emergencies,Special Services. A total of four child records and one vendor record was reviewed. The inspection started at 11:15 am and concluded at 12:15 pm.
Information gathered during the inspection determined non-compliance with applicable subsidy requirements or law and violations were documented on the violation notice issued to the program.

Based on record reviews the vendor failed to ensure that caregivers shall undergo TB screenings at least every two years from the date of the initial screening, or more frequently if recommended by a physician.
Evidence:
The vendors TB screening was dated 10/4/2019.
Plan of Correction: The vendor will have a TB screen conducted and will ensure that they are conducted every two years.

Based on record reviews the vendor failed to ensure that certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care, was current.
