Inspector Notes
Thank you for your assistance during this unannounced monitoring inspection. This inspection was conducted on May 25, 2022 from 10:30 am through 12:45 pm. Upon arrival, there were 18 children, ages 8 months through 5 years of age. Three children and three staff records were reviewed and updated. No medication is being given at this time.
Areas observed were: handwashing/diapering procedures, free choice activities, infant feeding, lunch and nap. Areas of non-compliance may be found on the violation notice.
Please complete the plan of correction and return to the licensing inspector within 5 business days.

Violations
3Evidence:
1. A box of Clorox wipe and 5 cans of Air Freshener were observed on a shelf outside of a classroom. Children walk past the shelving unit to use the bathroom.
2. One can of Air Freshener and Vaseline were observed above on a shelf, above the toilet , in a bathroom used by children.
3. The director confirmed the items were not in a locked place.
Evidence:
1. One outlet in Classroom #4 was observed not having an electrical cover.
2. The staff confirmed there was not an electrical cover on the outlet.
Evidence:
1. The diaper changing pad in the infant room was observed to have small tear.
2. The director confirmed the changing pad was torn in one area.
