During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on April 29, 2025, as a part of the licensure period. The on-site inspection began at 11:42 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 19 children present and 3 staff. The inspector reviewed five children?s and three staff records. This inspection included document review tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 12. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premised.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #3, employed for one month, did not have documentation of requesting a child abuse and neglect search from one state.
Plan of Correction: Director submitted application on 5.7.2025 in WV State for employee background check and for CPS.

The staff-to-children ratio for children ages birth through 16 months is 1 staff member for every 4 children.

There were 7 infants, ages 6 months through 1 year, in the care of 1 staff member from 11:43 am until 12:06 p.m.
Plan of Correction: Due to employee illness we were without a worker and will return on 5.13.2025

There were 12 children, ages 2 through 4, with one staff member, from approximately 11:423 a.m. through 12:06 p.m.

Cribs shall be identified for use by a specific child.

Four cribs in the infant room were not labeled in a manner to reflect use by a specific child.
Plan of Correction: Cribs were labeled but the label fell off. Cribs are now labeled with child's name.
Standard 8VAC20-780-570-E
Prepared infant formula shall be refrigerated, dated and labeled with the child's name.
One bottle was refrigerated without the child's name and date.
Plan of Correction: Child's bottle was labeled with the name and date but we did not notice it fell off. Label and date check is done at dropping off.
