During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on April 29, 2025, as a part of the licensure period. The on-site inspection began at 11:42 a.m. and ended at 1:00 p.m. The inspector reviewed compliance in the areas listed above. There were 19 children present and 3 staff. The inspector reviewed five children?s and three staff records. This inspection included document review tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 12. A POC submitted after this date will not appear on the public website.

Staff #3, employed for one month, did not have documentation of requesting a child abuse and neglect search from one state.
The staff-to-children ratio for children ages birth through 16 months is 1 staff member for every 4 children.
There were 7 infants, ages 6 months through 1 year, in the care of 1 staff member from 11:43 am until 12:06 p.m.
There were 12 children, ages 2 through 4, with one staff member, from approximately 11:423 a.m. through 12:06 p.m.
Four cribs in the infant room were not labeled in a manner to reflect use by a specific child.
One bottle was refrigerated without the child's name and date.
