During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 06/24/2025 in response to a complaint received by the licensing office on 06/23/2025 relating to allegations of reports, supervision, and procedures for emergencies. The inspector reviewed one children?s record on-site.

The preponderance of evidence gathered during the investigation supports the allegations regarding reports and procedures for emergencies; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/24/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of the occurrence.

Child A (Age 2) sustained an injury in the two year old classroom to their head on 6/18/2025. There was no documentation available for review to document the occurrence of the injury.
Plan of Correction: The center administration will implement improved procedures to ensure that all documentation is properly stored and readily available for future review.
Standard 8VAC20-780-80-B-2
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

On 6/18/25 Child A (Age 2 years) sustained an injury to their head at the facility and was later evaluated at an urgent care facility that evening. The parent of child A notified the facility of the urgent care visit to facility management on 6/20/25. The center did not report the incident to the department within two business days as required.
Plan of Correction: The center administration will ensure that all incidents are reported to the department within two business days if medical treatment is administered or if a parent seeks medical attention.
