Inspector Notes

An unannounced renewal monitoring inspection was conducted in the center from 10:00 AM until 11:50 AM on June 5, 2023. At the time of entrance, 15 children were in care with 4 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed playing outside and engaged in group activities and watching an educational video.
Repeat violations were found and are identified within this report. Areas of non-compliance are identified on the violation notice.

Standard 8VAC20-780-340-D
Based on staff record review, observation and staff interviews, the licensee did not ensure there shall be in each grouping of children at least one staff member who meets the qualifications of a program leader or program director.

Evidence: Staff #5, was the designated program lead teacher for the infant room and was observed alone with 3 infants. Staff #5 does not have documentation of the training/education required to meet the program lead position. Center owner confirmed the missing documentation.
Plan of Correction: Staff completed remaining fie hours for lead teacher qualifications. Director obtained documentation of training certificates and High School Diploma
Standard 8VAC20-780-40-K
Based on review of center documentation and staff interviews, the licensee did not ensure there were written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Evidence: Center owner confirmed there was not a written procedure available for review.
Plan of Correction: Owner will write a procedures for prevention of topics discussed in 8VAC20-780 -(2)-40-K.
Standard 8VAC20-780-60-A
Based on record review and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contained all required information.

Evidence: The records for Child #1, #2, #3, #4, and #5 did not include the previous childcare / school attended.
Plan of Correction: Director will contact parents for information on previous daycare attended and document in the child's record
