Inspector Notes
An unannounced monitoring inspection was started in the center on November 29, 2023 from 2:30 PM until 4:35 PM and concluded on November 29, 2023. At the time of entrance, 12 children were in care with 4 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during rest period, dancing with a video, playing freely and infants were observed eating.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
8Evidence: In the infant room in an unlocked cabinet there were containers of disinfectant spay and wipes. These hazardous chemicals were observed, and they were labeled "keep out of reach of children" and at least one other statement "caution", "flammable" and "warning".
Evidence: The diaper pad/ surface was not cleaned with soap and at least room temperature water and sanitized after each use. Staff #3 was observed conducting diaper changes fand only wiped down the diaper pad with a sanitizing wipe. Soap and water were not used to clean the surface and it was not sanitized as required. Sanitized is defined as sprayed with a disinfectant /sanitizing solution and allowed to air dry or wiped down after two minutes.
Evidence: Center Director confirmed medication (Epi-Pen) belonged to Child #1 and it was not labeled with the child's name, the dosage amount, and the time or times to be given.
Evidence: In the toddler room there were 1 medication (Epi-Pen) observed stored in an unlocked first aid box attached to the wall.
Evidence: Child #3 and Child #2 were observed eating snack in their highchairs and center staff were observed only wiping down the high tray with a sanitizing wipe. The highchairs were not sanitized as required. Sanitized is defined as sprayed with a disinfectant /sanitizing solution and allowed to air dry or wiped down after two minutes.
Evidence: Child #3? was observed drinking a bottle and it was not labeled or dated.
Based on record review and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contained all required information.
Evidence: Center Director verified the following items were missing from the children?s records.
1.Records for Child #1, #2, #3, #4, and #5 did not include the previous childcare / school attended.
2. The records for Child #1 did not include home address, employment location or employment phone number for one parent and no employment phone number for the 2nd parent.
3. The record for Child #2 did not include home address, home phone number, employment location or employment phone number for one parent and the address for the second emergency contact listed.
4. The record for Child #3 did not include the employment phone numbers for both parents and the addresses for both emergency contacts listed
Evidence: Center Director verified the following items were missing from the staff records;
1.Staff #1, #4 and #5 did not have documentation of any health-related problems that my interfere with the job.
2. Staff #2 did not have documentation of two references conducted prior to hire.
3. Staff #4 date of hire was not documented
