Inspector Notes
An unannounced monitoring inspection was started in the center on May 22, 2024 from 12:55 PM until 3:20 PM and concluded on May 23, 2024. At the time of entrance, 13 children were in care with 4 staff members. The sample size consisted of 5 childrens records and 5 staff records. Children were observed during rest period, eating preparing for and eating snack and playing freely.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

9Evidence: Child #1, who has an (Epi-Pin) was observed at the center and there was not a person at the center who was trained to administer the emergency medication. Staff #5 confirmed they were the only trained staff working at the center during the inspection and they had left for a lunch break.
Evidence. Staff #5 confirmed they are the only person trained in DHO during the inspection and they were not at the center when the Licensing Inspector arrived.
Evidence: Staff #5 confirmed they did not have an updated injury prevention plan.
Evidence: Three out of thirteen children were observed sleeping without a second cover (top pr bottom).
Evidence: Five injury reports reviewed and were missing information to include the
future action to prevent injury, the date, time and how the parent was notified of the injury.
Evidence: Potato chips were served as the afternoon snack to the toddlers, two year old and preschool children and therefore the snack did not meet the USDA guidelines. As required two food groups shall be served as snack and food items shall meet the USDA requirements.
Based on observation and provider interviews, the licensee did not ensure that tables and highchair trays shall be sanitized before and after each use for feeding.
Evidence:
1. Three infants were observed eating snack in highchairs and the three highchair trays were not sanitized before or after use.
2.Toddlers were observed eating snack at a table and it was not sanitized prior to eating.
Sanitized is defined as sprayed with a disinfectant /sanitizing solution and allowed to air dry or wiped down after two minutes.
Based on record review and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contained all required information.
Evidence: Staff #5 verified the following missing items from the childrens records.
1.The record for Child #1 did not include the name, home address, employment location or employment phone number for one parent listed.
2. The record for Child #2 did not include home address, employment location or employment phone number for one parent listed.
Based on record review and staff interviews, the licensee did not ensure each staff record contained all required documentation.
Evidence: Staff #5 verified the following missing items from the staff records;
1.Staff #1 and staff #4 did not have a copy of their diploma/transcripts.
2. Staff #2 did not have documentation of two references conducted prior to hire and Staff #3 did not have documentation of the 2nd required reference.
