During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on June 26, 2025 and completed on July 23, 2025 in response to a complaint received by the licensing office on June 9, 2025 relating to allegations of programs and terms of the license. The inspector reviewed 1 child records and 1 staff records electronically on July 8, 2025 . This inspection included document reviews, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be not valid for programs but valid for terms of the license. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 11, 2025. A POC submitted after this date will not appear on the public website.

The center provided care for Child#1, at 4 weeks and the term of its license is 6 weeks-5 years old.
Plan of Correction: The center now provides care only to children 6 weeks to 5 years old.
