Inspector Notes
An unannounced, on-site focused monitoring was initiated and completed on November 19, 2025 as a part of an Intensive Plan of Correction (IPOC). The on-site inspection began at 10:54 am and ended at 1:04 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 5 staff supervising. The inspector reviewed 4 children's records and 3 staff records onsite. This inspection included document review, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
