Inspector Notes
An unannounced monitoring inspection was conducted on February 5, 2024 between the hours of 11:45 am through 12:45pm. There were a total of four children (11 points) observed with one caregiver. A total of four children's files and two staff/household member's files were reviewed. The children were engaged in the following: toy play and other organized activities. 1. Caregiver #1 did not have current CPR certification. Caregiver #1's CPR certification expired on 01/15/2024.

1. A substitute provider was taking care of four children at the family day home. The substitute provider did not record and sign the time of arrival for the day.

1. Child #2's record did not contain two addresses for emergency contacts.

1. Child #1 has emergency medication on site. Child #1's record did not contain written authorization from the parent to administer medication.
2. Caregiver #1 confirmed that written parent authorization was not obtained from the parent.
