Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on February 25, 2025, as a part of the licensure period.The on-site inspection began at 9:30am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 16. The inspector reviewed six children?s records and three caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-4
The family day home did not complete an out of state criminal background history records request, out of state child abuse and neglect search request, and an out of state sex offender registry search in which the individual resided in the preceding five years and prior to employment.
An out of state criminal background history records request, out of state child abuse and neglect search request, and an out of state sex offender registry search was not conducted for Caregiver #2, who has been employed for one month and was working with children at the time of the inspection.
Plan of Correction: The provider will conduct an out of state criminal history records request, a child abuse and neglect search request , and a sex offender registry search for Caregiver #2. Caregiver #2 was never left alone with children and always supervised by the provider.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.
There was no documentation of a physical examination available for Child #1, who had been in care for six months.
Plan of Correction: The parent of Child #1 will be contacted today to obtain a copy of the current physical examination and ensure that it is signed by a physician.
Standard 8VAC20-800-680-A
Caregivers shall wash their hands with liquid soap and warm running water before feeding or helping children with feeding.
Caregiver #2 did not wash their hands with liquid soap and warm running water before feeding a child a bottle.
Plan of Correction: Caregiver #2 will be retrained on handwashing procedures and to ensure that they wash hands before feeding children.

Caregivers shall ensure that children's hands are washed with liquid soap and warm running water before eating.
Caregiver #2 did not ensure that four children's hands were washed with liquid soap and warm running water before eating snack.
Plan of Correction: Caregiver #2 will be retrained on handwashing procedures for children. Caregivers will ensure to wash children's hands with liquid soap and warm running water before children eat.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.
Record for Child #3 did not have a parent's signature for the annual review of the emergency contact information. Child #3 has been in care for seventeen months.
Plan of Correction: The parent of Child #3 will review the emergency contact information on the child's record and sign the record as acknowledgment.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.
There was no documentation of immunizations for Child #1, who was in care during the inspection and had been in care for six months.
Plan of Correction: The parent of Child #1 will be contacted today to obtain a copy of the current immunization records. The provider will ensure to obtain immunization records prior to any child attending the family day home.
