Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on September 9, 2025, as a part of the licensure period. The on-site inspection began at 10:10am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 20. The inspector reviewed six children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of 16 clock hours of training annually.
Caregiver #1 obtained 3 of the 16 clock hours of annual training. Caregiver #2 did not obtain any hours of annual training.
Plan of Correction: Caregiver #1 and Caregiver #2 will complete the hours of annual training by the end of the month. The caregivers will ensure to obtain the minimum amount of hours of annual training each year by taking on line trainings.
Standard 8VAC20-800-340-C
The bathrooms shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap, and paper towels.

The bathroom did not contain paper towels.
Plan of Correction: Paper towels will be added to the bathroom.
Standard 8VAC20-800-780-B
Repeat Violation

Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents' signed acknowledgment of the review.

The record for Child #4 did not have the parent's signature for the annual review of the emergency contact information. Child #4 has been in care for 23 months without any annual review.
Plan of Correction: The provider will have the parent of Child #4 to review the emergency contact information and sign the record as acknowledgment. Moving forward, the provider will ensure to review the records on a monthly basis to ensure that annual reviews of the emergency contact information are being conducted.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. Child #2,15 months old, last documented immunization was 10 months ago. Child #6, 18 months old, last documented immunization was 18 months ago.
Plan of Correction: The provider will ask the parents of Child #2 and #6 to provide a copy of the current immunization records for each child and document the record.
