Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 11, 2026, as a part of the licensure period. The on-site inspection began at 10:10am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was 18. The inspector reviewed six children?s records and three caregiver/household member(s) records on-site.

This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-170-A
The provider shall obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis(TB) Screening .

The record for Caregiver #3, employed for two months, contained a negative TB report dated after the time of hire and after coming in contact with children. Caregiver #1 confirmed that Caregiver #3 did not submit a TB screening at the time of hire and completed one after coming in contact with children.
Plan of Correction: Moving forward, the provider will ensure to obtain a negative TB screening at the time of hire and before the caregiver comes in contact with children.
Standard 8VAC20-800-60-B
A child's record shall contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached and the policy number of the Child's medical insurance.

The record for #3 did not contain a second emergency contact and the policy number for the child's medical insurance. Child #3 has been in care for nine months.
Plan of Correction: The provider will obtain a second emergency contact and the policy number of the child's medical insurance from the parent.
