Inspector Notes

The licensing inspector initiated an unannounced inspection on 08/29/23 and concluded on 09/13/23 in response to a complaint received by the department on 08/21/23 for allegations regarding parent communication and supervision. Interviews and observations were conducted relating to the allegations. The information gathered during the investigation support the allegation in part, so the complaint is determined to be "valid." Areas of non-compliance are identified on this Violation Notice.

Based on interview, the center failed to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Evidence:

1. The parent of child 1 stated on 8/16/23 after pick up at 3:30 PM, the child she noticed a scratch on his face. The parent stated she did not receive an injury report.
2. Staff 1 stated an injury report was not sent home with the parent regarding the injury on 8/16/23.
Plan of Correction: Responsible party: Leadership, Classroom Teams
1. Immediately following the missed reporting, all classroom teams were provided with daily communication notebooks to document behaviors/needs from throughout the day that should be monitored by staff transitioning into a classroom for relief coverage to avoid details being missed in communication during this routine time of transition.
2. Policy for response to incident and/or injury has been updated; staff are to call in for support following a circumstance that requires a write up, allowing the report to be immediately written while the children are observed and engaged with by the support staff.
