During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/17/2024 and completed on 10/17/2024. The on-site inspection began at 12:20 PM and ended at 3:00 PM. The inspector reviewed compliance in the areas listed above. There were 58 children present and 13 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/28/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
A spray bottle labeled bleach water was hanging above the changing table area in the Unique Unicorn waddler classroom. Container of Clorox wipes with warning labels, ?Keep out of reach of children,? was on an open shelf, out of reach of children, in the Dizzy Dinos three-year-old restroom.
Plan of Correction: The entire team was reminded upon violation to lockup all cleaning supplies; the two classrooms immediately remedied the issue.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.
Medication for Child 6 was onsite, but the medication authorization had expired three weeks ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed and the medication was not returned or disposed of as required.
Plan of Correction: The parent was immediately notified of the need to pick up medication and did so.
