Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on May 3, 2021 and concluded on May 3, 2021. The provider was contacted by telephone to initiate the inspection. There were nine children present and two caregivers. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on review of caregivers training and interview, the provider did not ensure Caregivers who work directly with children in addition to preservice and orientation training annually attend at least 16 hours of training, to include the department's health and safety update course.

Evidence:

1. The most recent documentation of completion for the annual health and safety training for caregiver #1 was dated 12/17/2019. There was no documentation of completing the health and safety training for the 2020 year.
2. Caregiver #2 completed Virginia preservice on 07/19/2017 and had until 03/01/2020 to complete the training, and then every 12 months thereafter. Caregiver #2 did not have documentation of completing the health and safety training.
3. The provider stated the updated health and safety training had not been completed in 2020.
Plan of Correction: Per the provider: we will complete the health and safety training annual going forward.
