Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on October 6, 2021 and concluded on October 6, 2021. There were five children present with two caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of caregivers training and interview, the provider did not ensure Caregivers who work directly with children in addition to preservice and orientation training annually attend at least 16 hours of training, to include the department's health and safety update course.

Evidence:

1. Caregiver #2 completed Virginia preservice on 07/19/2017 and had until 03/01/2020 to complete the training, and then every 12 months thereafter. Caregiver #2 did not have documentation of completing the health and safety training.
2. The provider stated the updated health and safety training had not been completed.
