Inspector Notes
A renewal inspection was conducted on August 11, 2023 from approximately 1:50 pm to 4:00 pm. There were 10 children present, ranging in ages 1 month to 5 years with two caregivers supervising for a total of 23 points. A total of five child records were reviewed and the records for the two caregivers and one adult household member were reviewed.
During the inspection, the children were observed during rest time, having an afternoon snack, and engaged in free play. The caregiver's interactions with the children were positive.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only. Violations
4Evidence:
The provider was not able to locate the license during the inspection.
Evidence:
1.The changing table had visible brown stains and coffee beans were observed underneath the changing pad.
2. During an interview, the provider acknowledged spilled coffee beans were under the changing pad and the changing table was not clean.
Evidence:
1.The diaper changing pad on the changing table had tears making it an absorbent surface.
2. Children were observed being diapered on the torn changing pad.
Evidence:
1. The practice evacuation drills documented for 2023 did not include an evacuation drill for July 2023.
2. During an interview, the provider acknowledged the last drill was conducted on 6/19/2023.
