Inspector Notes

A monitoring inspection was initiated on 12/13/2023 and concluded on 12/13/2023 from 2:50 PM to 4:10 PM. There were 36 children present, ranging in ages from six to eleven, with six staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the six staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review the center failed to obtain documentation of a physical examination before the child's attendance or within 30 days after the first day of attendance.

Evidence:

The record for child 4 did not contain documentation of a physical. The first date of attendance for child 4 was 6/19/2023.
Plan of Correction: The physical was obtained on 12/13/2023 and added to child 4's file. In the future the director will ensure each child's record is complete per the standards.
