Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on February 17, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were sixteen children present, ranging in ages from infants to preschool, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 1 of 2 children records has an immunization record prior to child beginning attendance.

Evidence: The record for child #2 (start date 2/7/2022) did not have an immunization record.
Plan of Correction: Mom brought in the immunization record.
Standard 8VAC20-780-330-B
Based on observation, facility did not ensure there is sufficient resilient surfacing under and around climbing equipment on the outside playground.

Evidence: A plastic little tikes slide was observed on the playground placed with the steps and one side of the slide on asphalt. There was not resilient surfacing under and at least 6' around the perimeter of the piece of climbing equipment.
Plan of Correction: Remove slide equipment until we find a safe plan for the ground.
