Inspector Notes

A monitoring inspection was conducted on April 18, 2023. There were twenty-two children present, ranging in ages from infants to five years, with fivestaff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure there is an annual approval from the health department.

Evidence: The last health inspection report available for review was completed August 25, 2021 which is more than a year old.

Based on interview, facility did not ensure there are required written procedures.

Evidence: Program Director stated there is not written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Based on observation and review, facility did not ensure pillows and filled comforters are not used by children under two years of age.

Evidence: The following was observed in the infant room:
1. Child #3 (age 15 months) was observed napping with a pillow.
2. Child #4 (age 13 months) was observed napping with a filled comforter.
