Inspector Notes

An unannounced monitoring inspection was conducted on July 24, 2023 from approximately 11:10 AM to 12:30 PM. There were twenty-six children present with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 1 of 4 staff have an updated TB test or screening every 2 years.

Evidence: The record for staff #2 had a most recent TB test/screening dated 8/18/2020 which is more than 2 years old.

Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition.

Evidence: A drawer cover was missing from one of the drawers under the counter in the infant room. Director confirmed the cover had come off and needed to be repaired/replaced.

Based on observation and review, facility did not ensure there is at least 1 staff per 4 children when the children are birth up to 16 months old.

Evidence: At approximately 11:15 am, staff #4 was present with 7 children in the infant room. Child #4, age 4 months, was the youngest in the room. Staff reported a staff member had called out on this morning leaving them out of ratio.
