Inspector Notes

An unannounced complaint inspection was conducted on June 3, 2024 from approximately 12:50 pm to 2:00 pm in response to allegations involving programs, special care provisions and special services. Three staff were present supervising twenty-four children ranging in age from infants through preschool. Licensing Inspector reviewed standards related to staffing/supervision, daily activities, diapering procedures, napping, nutrition, and recordkeeping. Based on observations and interviews conducted, there is not a preponderance of evidence to support the allegations therefore the complaint is deemed not valid. Violations were found that were not reported. Please see the violation notice for further details.

Based on observation and interview, facility did not ensure there is at least 1 staff for every 4 children age birth up to 16 months.

Evidence: Staff #1 was observed present with 8 infants and toddlers in the infant room. Staff stated there was another staff working in the room but that individual was on break. Staff #2 returned to the room and stated she had taken a 30 minute break.

Based on observation, facility did not ensure there is at least 12" of space between occupied cots.

Evidence: Two children were observed napping on cots in the infant room. There was approximately 4" of space between the cots.

Based on observation and interview, facility did not ensure cribs are used for children under 12 months of age.

Evidence: Child #3 (10 months old) was observed napping in a infant rocker seat on the floor. Staff stated that child did not like to sleep in a crib so child sleeps in infant seat or is held for napping.
