Inspector Notes

An unannounced monitoring inspection was initiated and completed on December 5, 2024. This inspection was conducted by licensing staff using an alternate remote protocol. The inspection began at 2pm and ended at 3:15pm. The inspector reviewed compliance in the areas listed above. There was one child present and one caregiver; the point total was 3. The inspector reviewed 2 children?s records and the caregiver and household member electronically on December 5, 2024. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The provider is required to have employees undergo a background check every five years. The most recent central registry checks for both provider and household member are expired.
Plan of Correction: The provider responded with the following: The provider and household member will obtain up to date central registry checks for Virginia and place the results in the case record.
Standard 8VAC20-800-180-A
The provider shall obtain for each adult household member a
current Report of Tuberculosis Screening form, in accordance with the
requirements in 8VAC20-800-170, every two years from the date of the first
screening. The household member has a tuberculosis screening which expired on November 19, 2023.
Plan of Correction: The provider responded with the following: The household member will obtain an up to date tuberculosis screening and place it in their file.
