Inspector Notes
A renewal inspection was conducted on 02/03/2023. There were 5 children present and 2 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 2children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition.
Evidence:
The wooden border surrounding the playground equipment had a section that was broken and splintered.

Plan of Correction: The wood will be sanded to smooth out rough edges and/or replaced.

Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.

There was a Little Tykes plastic slide on the playground that did not have at least 6 feet of resilient surfacing on all sides including the fall zones of the slide.

Plan of Correction: The slide may have moved out of place due to the weather. The small slide will be remove.
