Inspector Notes

An unannounced, on-site renewal inspection was initiated on December 20, 2024 and completed on December 20, 2024. The on-site inspection began at 10:15am and ended at 11:54am. The inspector reviewed compliance in the areas listed above. There were 20 children present and 5 staff. The inspector reviewed 5 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/3/2025. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1, under two years old has been in care for 9 months did not have additional immunizations in their record.

Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. The following maintenance issues were observed: The eating area floor had a cracked tile and the infant and toddler room had various areas with peeling paint within reach of children.

In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Staff #1, who is qualified as an aide was supervising two infants alone in the classroom.
