Inspector Notes

An unannounced, on-site monitoring inspection was initiated on June 3, 2025 and completed on June 3, 2025. The on-site inspection began at 10:40pm and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (6/16/2025). A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1, under two years old has been in care for 9 months did not have additional immunizations in their record.
Plan of Correction: Letters were sent out to ensure we meet guidelines for immunizations.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in an operable condition. The following maintenance issue was observed. The napping mat used for child #1 was ripped exposing the interior fabric.
Plan of Correction: The staff were retrained on licensing procedures for sleeping mats.
