Inspector Notes

An unannounced renewal inspection was conducted on 12/15/2021. There were 8 children, ages 19 months-3 years totaling 18 points, and 2 caregivers present during the inspection. The inspector observed the following: learning activities and free choice activities. The inspector reviewed 5 children?s records, 2 caregiver records, 2 household member records and 2 pet records. Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the provider. Based on observations and discussion, the provider failed to ensure that the bathroom shall contain toilet tissue, liquid soap and paper towels.

Evidence: There were no paper towels available for children's use in the bathroom.
Plan of Correction: Paper towels were placed in the bathroom during the inspection.
Standard 8VAC20-800-800-B
Based on document review and discussion, the provider failed to review the emergency plan at least annually and update the plan as needed; the provider shall document the review in writing.

Evidence: The most recent review of the emergency plan was documented 06/19/2020.
