Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/16/2024. The on-site inspection began at 10:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were (2) resident children and (1) child in care and (1) caregiver; the point total was (7). The inspector reviewed (1) child?s records and (1) caregiver record on-site on 9/16/2024. This inspection included document review, tour of the home and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation, the license and any other documents required by the
Superintendent were not posted in a conspicuous place on the licensed premises.
Evidence:
1.The last inspection dated 4/03/2024 was not completely posted. The provider had the summary and only one page of the violation report posted.
Plan of Correction: I will make sure to post the entire inspection today.

Repeat Violation
Based on review of the children's records, the parents were not provided in writing the
required information before the child's first day of attendance.

1. Child #1 did not have the policy for administrating medication in the file signed by the parent..
