Inspector Notes

A complaint inspection was conducted on November 7, 2022 in response to a complaint that was received by the department on October 6 2022, regarding allegations in the care of the children at the center. Staff were interviewed, documentation was reviewed, children?s records were reviewed, and classroom observations were completed. The evidence gathered during the inspection determined non-compliances with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation was discussed with the center?s Director at the exit interview.

Based on record review and interview, it was determined that the center did not ensure that each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
1. Child #1 (date of enrollment 10/10/2022) did not have the information of two designated people to call in case of an emergency.
2. Child #2 (date of enrollment 10/10/2022) did not have the information of two designated people to call in case of an emergency.
3. Staff #1 confirmed that Child #1 and Child #2 did not have two designated people to call in of an emergency to their records.
Plan of Correction: The center responded with the following: Staff #1 will obtain the emergency contact information this week from the parent and have it documented to the children?s records.
