An unannounced inspection was initiated on January 30, 2026 and completed on February 4, 2026 in response to a complaint received by the licensing office on January 29, 2026 relating to allegations of ratio, supervision, care and protection and emergency procedures. The inspector reviewed 6 children?s record and 4 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/20/26. A POC submitted after this date will not appear on the public website.

Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member who is certified in Medication Administration. The center had children?s medication on site and the medication administration training certificates for the staff working were expired.

When staff are supervising children, they shall always ensure their care, protection, and guidance.
On January 28, 2026, child #1 (15 months) who is diagnosed with a nut allergy and ingested food at the center that contained nuts.
Staff #1 was aware of a nut allergy in the center and served food containing nuts to children.
Several staff did not have access to emergency medication or emergency contact information.

The staff-to-child ratio for children ages birth to 16 months whenever children are in care is one staff for every 4 children.
On January 26, 2026, the infant ratio was 2 staff for 9 infants.
On January 27, 2026, the infant ratio was 2 staff for 9 infants.
On February 4, 2026, the infant room ratio was 3 staff for 14 infants.

The staff-to-child ratio for children ages 3 years to school age eligible whenever children are in care is one staff for every 10 children. On February 4, 2026, the ratio in the pre-k room where the youngest child was three years old was one staff for 12 children.

For each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child and contain all required information. Five infant daily records were reviewed, and three records did not indicate the time the infant slept and two records did not indicate the amount of food consumed and the time.

The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided that the staff member has current medication administration certification training. On January 28, 2026, staff #3 administered a prescribed epinephrine medication to child #3 and did not have certification in medication administration.
