Inspector Notes
An unannounced monitoring inspection was initiated on 03/14/2025 and completed on 03/18/2025 in response to a self-report received by the licensing office on 03/13/2025. This inspection included: document review (i.e. written statements of incident, and staff training certificates ); tour of the facility; and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
A three year old child (Child #1) was left outside on the center playground for approximately 5 minutes. This incident occurred on 03/12/25, between approximately 4:30 pm to 4:35 pm. Staff #1 failed to conduct a name-to-face check when entering the building prior to the return to the classroom.

Plan of Correction: Retraining will be conducted for all staff. Staff #1 was terminated from employment and Staff #2 and Staff #3 were placed on suspension.

The operational responsibilities of the licensee shall include ensuring that the center's activities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.
