Inspector Notes

An unannounced monitoring inspection was conducted on October 25, 2023, from 11:40am to 1:30pm with the provider. There were nine children, ages 9 months, 11 months, 17 months, 2, 2, 2, 3, 4 and 4 years old, were in the care of three caregivers for a total of 23 points. The children were observed eating lunch which consists of chicken, rice, salad, cooked carrots, apple slices, and milk. Four children?s records were reviewed. The provider, two household member, and two staff records were reviewed. There were no medications on site to be reviewed. The provider has current certification in CPR and First Aid. The first aid kit, flashlight, and battery-operated radio were observed.

Standard 8VAC20-800-100-A
Based on a review of four children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child A 's record was missing documentation of a physical examination within 30 days after the first day of attendance. The child's first day of attendance was1/17/2023.
Plan of Correction: The provider requested the missing documentation from the parent. Once it is received the documentation will be placed in the child's record.
Standard 8VAC20-800-120-A
Based on a review of two assistant's records, the provider failed to ensure that all assistant's records contained the required information. Evidence: Assistant A and assistant B's records were missing documentation of two references.
Plan of Correction: The provider will obtain the missing documentation from the staff members and placed it in their records.
Standard 8VAC20-800-50-A
Based on a review of documentation, the provider did not ensure that there was a written record of children in attendance for each day. Evidence: There were 7 children signed in on the written attendance sheet and 9 children were physically present.
Plan of Correction: The provider spoke to the assistance about recording the arrival and departure of children as it occurs during the inspection.
Standard 8VAC20-800-60-B
Based on a review of four children's records, the provider failed to ensure that all children's records contained the required information. Evidence: Child B's record was missing documentation of an annual review and policy for administration of medications.
Plan of Correction: The provider will request the missing documentation and update the child's record.
