Inspector Notes
A monitoring inspection was completed from 11:45 a.m. until 1:30 p.m. as a result of a self reported incident. Two staff records were reviewed, interviews were completed, and documents were reviewed with another agency.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews and record review, the center failed to ensure when staff are supervising children that staff shall always ensure their care, protection, and guidance.
Evidence:
1. Staff #1 and staff #2 stated that child #1, four years old, was inside the classroom for approximately five (5) minutes alone and unsupervised.

Standard 8VAC20-780-340-F
1. Interviews with staff #1 and #2 determined that child #1, four years old, was left out of sight and sound supervision of staff for approximately five (5) minutes. Both staff stated that they were unaware child #1 was not on the playground and was alerted by staff #3.
2. Review of records indicated that class A went outside at 11:23 am and class B came inside at 11:30 am. Staff #3 stated at 11:30 am she was taking her class (B) inside from the playground through classroom A. Child #1 was observed in classroom A, alone, near the window/play kitchen area. Both staff #1 and staff #2 confirmed this timeframe as they observed child #1 on the playground for approximately two to three minutes after arriving on the playground at 11:23 am.
