Inspector Notes

An unannounced monitoring inspection was completed from 10:20 until 1:00pm. There were 77 children and 13 staff supervising. The assistant director and two corporate staff representatives were also present.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 staff records and 5 children?s records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on record review, the center did not have the required documentation for TB screenings.
Evidence:
1. Staff #4 (DOH 9/18/23) did not have a TB screening documented. Staff #5 (DOH 3/7/23) had a TB screening 3/7/23. Documentation of a negative TB screening shall have been completed within the last 30 days of the date of employment.
Plan of Correction: Staff will get their documentation of a negative TB screening within the last 30 days of the date of hire.
