Inspector Notes
An announced monitoring inspection was initiated on 7/28/25 and completed on 7/28/25 in response to a self-report received by the licensing office on 7/10/25 relating to supervision. This inspection included document review, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

2Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff #1 failed to ensure the care, protection, and guidance of child #1 (4 year old) for approximately 2 minutes on 7/9/25 around 3:30pm. The child was left alone in the classroom, while in the restroom, when the group transitioned to combine with another class. Staff #1 was not aware the child was not with the group upon transition, and upon arriving back to the classroom found the child alone in the classroom.
