During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 8/11/2025. The on-site inspection began at 12:20 pm and ended at 2:05 pm. The inspector reviewed compliance in the areas listed above. There were 38 children present with 5 staff. The inspector reviewed 5 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

The center is required to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. There were no results or documentation to show that a request for an Out of State Child Abuse and Neglect Search Request had been requested prior to employment or service for Staff #2. The request was overdue by more than 30 days.
Plan of Correction: This was an oversight. The process to request an out of state abuse and neglect registry search for staff #2 was initiated on 8/11/2024. Upon receipt, the results will be placed in the staff record and a copy emailed to the licensing inspector.

Staff #2 received cleared results on 8/13/2025.
