Violations

1
Standard 8VAC20-780-60-A
Based on record review and interview, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: Child #6's physical exam confirms that the child has a diagnosed food allergy. Center management confirmed that they had not obtained an allergy care plan for the child.
Plan of Correction: I will contact the family and provide them with the allergy care plan form along with the medication forms if the doctor confirms that the child needs to have medication kept on site.
