Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 11:20am and 2:30pm. There were 8 classrooms observed with a total of 75 children with 12 staff within the supervision guidelines. The children were observed playing outside on the playground, eaching lunch, washing hands and transitioning to naptime. A complete inspection of the physical plant, children and staff records, medications and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-780-350-B-1
Based on observation, the center did not ensure that there was 1 staff to 4 children for children age birth through 16 months.
Evidence: During the inspection, child #3 (12 months) and child #8 (14 months) were observed in the toddler classroom. There were ten children observed with two teachers.
Plan of Correction: The children are in transition this week. When the classroom went over 8 children, the teachers should have called the office to move children.
Standard 8VAC20-780-440-B
Based on observation, cots were not identified for use by each specific child.
Evidence: The cots in the Early Preschool classroom were not labeled to identify use for each specific child.
Plan of Correction: The teachers are being given new class lists and they will label all of the cots.
Standard 8VAC20-780-510-G
Based on a review of medications, medication was not labeled with each child's name.
Evidence: Child #8 had two emergency medications observed on-site. Both medications were not labeled with the child's name.
Plan of Correction: Corrected during the inspection.
Standard 8VAC20-780-510-P
Based on a review of medications, medication was not returned to parents or discarded within 14 days of it's authorization expiring.
Evidence: Child #9's authorization to administer emergency medication was signed by the parent on 6/1/2023 and expired on 6/15/2023. The medication was observed on-site during today's inspection.
Plan of Correction: We will have the parents fill out a new short-term authorization. If they want the medication to be kept on-site for more than 10 days it will get signed by a doctor.
