Inspector Notes

A monitoring inspection was initiated on 5/12/2022 and concluded on 5/12/2022. There were a total of 5 children ( 8 points) observed with 2 caregivers present. The children were engaged in the following: toy play, A total of 2 child records and 3 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on record review and provider interview, the provider has not obtained documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1, date of first attendance 1/3/2022 does not have documentation of a physical examination in the record.
The provider said she does not have the physical examination.
Plan of Correction: Provider will ask the parent for the physical examination record.
Standard 8VAC20-800-180-A
Based on a review of caregiver and household records, the provider has not obtained for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
Evidence: There were no current TB screening for the following caregivers and household members.
Caregiver#1 TB screening dated 2/24/2020
Caregiver #2 TB screening dated 8/30/2019
Household #3 TB screening dated 8/27/2019
Plan of Correction: Caregivers and household member will make an appointment get documentation of TB screening.
Standard 8VAC20-800-320-B
Based on observation, child-resistant protective covers were not installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence: Upon arrival the Inspector observed a 15 month old touching an electrical outlet on the wall located next to the childcare entrance door. The outlet was not covered with a protective cover.
Plan of Correction: Provider immediately place a cover on the outlet but the child removed it again. the provider will find a safe way to secure the protective cover on the electrical outlet.
