Inspector Notes

A monitoring inspection was initiated on 1/3/2023 and concluded on 1/3/2023. There were a total of 4 children (12 points) observed with 2 caregivers present. The children were engaged in the following: toy play and flash cards. The home, 2 child records, 2 caregiver/household member records, evacuation drills, emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of children records, a child's record did not contain all the required information to include medical insurance policy number and 2 emergency contact name, address and phone numbers.
Evidence: Child #1, date of first attendance 8/15/2022 does not have information on medical insurance policy number or a second emergency name, address and phone number.
Plan of Correction: The provider obtained the missing information. The provider will have the form completed in full prior to the child's fist attendance.
